The Achievement Center is celebrating 90 years of assisting children to reach above and beyond their therapeutic goals.
Since 1923, the center has expanded and grown to meet the needs of nearly 3,400 children annually with physical disabilities, developmental delays, emotional/behavioral problems, and autism spectrum disorders.

Blended Case Management Services
Blended Case Management (BCM) Services support the physical and behavioral health needs of children and adolescents. By addressing all areas of concern for the child/adolescent and family, BCM helps to build upon strengths, laying the foundation for more positive family interactions, skill building and healthy coping mechanisms.
Blended Case Managers help identify services to support your family and will coordinate access to get the help you need.
Blended Case Managers work closely with families to coordinate, access and follow through with the services needed for the child/adolescent and family. Working together, your BCM will help you build a responsive network of support. Finding what works best for you, your child/ adolescent, Blended Case Managers help find your family’s strengths and get help where you need it most.
